Camp sign-in and sign-out sheet template
Record each child’s arrival and pickup on one sheet per group, per day. Print a new sheet every morning and file it at the end of the day.
What the sheet covers
- The program, group, date and staff on duty
- Time in and time out for each child
- The adult who dropped off and the adult who picked up each child
- Staff initials at drop-off and pickup, and a column for notes
This sheet is a starting point, not legal advice. Check your province’s child care rules for what to record and how long to keep it.

Common questions
What should a sign-in and sign-out sheet include?
The date and group, each child’s name, the time in and time out, the adult at drop-off and at pickup, and staff initials. A notes column covers late arrivals and early pickups.
Who should fill in the sheet, staff or parents?
Either works. Many camps have staff write the times and initial each line, so the sheet stays readable and no line is skipped at a busy door.
How long should I keep sign-in sheets?
Follow your province’s child care rules and your insurer’s advice. File them by date so you can find a single day quickly.
Can I replace the paper sheet with an app?
Yes. Campix records each check-in and check-out with the time, the staff member and the adult at pickup, on every plan, including the free Solo plan.
To check children in and out from a phone, see Campix check-in software.
