Pickup authorization form template
Have each family name the adults who may pick up their child. Keep the signed form where staff can check it at pickup.
What the form covers
- The child, the program, and the parent or guardian
- Adults allowed to pick up, with their relationship and phone number
- Adults who may not pick up, and a place to attach a court order
- An optional pickup PIN, pickup rules, and a signature line
